Hefen und Schimmelpilze, bei Patienten ab 13 Jahren (Tabletten\/orale Suspension) bzw. ab 2 Jahren (Infusion), mit hohem Risiko f\u00fcr diese Infektionen; (Erwachsene oder ab 2 Jahren f\u00fcr IV) Fusariose: therapierefrakt\u00e4r oder Unvertr\u00e4glichkeit gegen\u00fcber Amphotericin B; Chromoblastomykose\/Myzetom: therapierefrakt\u00e4r oder Unvertr\u00e4glichkeit gegen\u00fcber Itraconazol; Kokzidioidomykose: therapierefrakt\u00e4r oder Unvertr\u00e4glichkeit gegen\u00fcber Amphotericin B, Itraconazol, Fluconazol. <strong>D: <\/strong><em><u>Tablette:<\/u> <\/em>Prophylaxe invasiver Pilzinfektionen\/Behandlung therapierefrakt\u00e4rer invasiver Pilzinfektionen: Initialdosis 300 mg (3&#215;100 mg) 2x tgl. am 1. Tag, danach 300 mg (3x 100 mg) 1x tgl.; unabh\u00e4ngig von der Nahrungsaufnahme. <em><u>Orale Suspension:<\/u> <\/em>Prophylaxe: 200 mg (5 ml) 3x tgl.; Einnahme zu Mahlzeiten oder mit Nahrungserg\u00e4nzungsmittel. Therapierefrakt\u00e4re invasive Pilzinfektionen (IFI)\/IFI und Unvertr\u00e4glichkeit: 400 mg (10 ml) 2x tgl. 200 mg (5 ml) 4x tgl. bei Patienten, die keine Mahlzeit\/Nahrungserg\u00e4nzungsmittel zu sich nehmen k\u00f6nnen<em>. <u>Infusionsl\u00f6sung<\/u>: Erwachsene: <\/em>1. Tag 300 mg 2x tgl., danach 300 mg 1x tgl. \u00fcber zentralven\u00f6sen Zugang \u00fcber mind. 90 Minuten verabreichen. Kinder von 2 bis &lt;12 Jahre: 1. Tag 2x tgl. 6mg\/kg, ab 2. Tag 1x tgl. 6mg\/kg bis zu einer max. Einzeldosis von 300mg. Jugendliche von 12 bis &lt;18 Jahren: 1. Tag 2x tgl. 4,5mg\/kg, ab 2. Tag 1x tgl. 4,5mg\/kg bis zu einer max. Einzeldosis von 300mg. Wen n\u00f6tig Steigerung bis auf 6mg\/kg. Vorsicht bei Patienten unter 60 kg: erh\u00f6hter Posaconazol-Spiegel und Toxizit\u00e4t m\u00f6glich. Besondere Sorgfalt bei Patienten \u00fcber 120 kg: reduzierte Posaconazol-Exposition m\u00f6glich. Dauer der Prophylaxe: abh\u00e4ngig von der Erholung nach Neutropenie oder Immunsuppression. Bei Patienten mit akuter myeloischer Leuk\u00e4mie oder myelodysplastischen Syndromen die Prophylaxe mehrere Tage vor dem erwarteten Auftreten einer Neutropenie starten und \u00fcber 7 Tage fortsetzen nach Anstieg der Neutrophilenzahl &gt;500 Zellen pro mm3. Die Therapiedauer richtet sich nach der Schwere der Grunderkrankung, der Erholung von einer Immunsuppression, dem klinischen Ansprechen. Kontrolle des therapeutischen Wirkstoffs empfohlen. <strong>KI: <\/strong>Gleichzeitige Gabe von Ergotalkaloiden, CYP3A4-Substraten, HMG-CoA-Reduktasehemmern. \u00dcberempfindlichkeit gegen\u00fcber Wirkstoff\/Hilfsstoffe. <strong>WH: <\/strong>Tabletten und orale Suspension sind nicht austauschbar. Bei Patienten mit \u00dcberempfindlichkeit gegen\u00fcber anderen Azolen, schwerer Leberfunktionsst\u00f6rung, arrhythmischen Erkrankungen, Elektrolytst\u00f6rungen, Glucose-Galactose-Malabsorption. Gleichzeitige Gabe von anderen durch CYP3A4 metabolisierten Arzneimitteln, Rifabutin, Rifamycin-Antibiotika, bestimmten Antikonvulsiva (Phenytoin, Carbamazepin, Phenobarbital, Primidon), Efavirenz, Cimetidin, Sirolimus, QTc-Intervall verl\u00e4ngernden Arzneimitteln. Die Anwendung zusammen mit Vinca-Alkaloiden ist zu vermeiden (Nutzen-Risiko-Abw\u00e4gung, ggf. Dosisreduktion des Vinca-Alkaloids). Pseudoaldosteronismus: Blutdruck und Kaliumspiegel \u00fcberwachen. <strong>DDI: <\/strong>Rifabutin, Efavirenz, Phenytoin, Fosamprenavir, Flucloxacillin, CYP3A4-Substrate, Mutterkornalkaloide, durch CYP3A4 metabolisierte HMG-CoA-Reduktasehemmer, Vinca-Alkaloide, Ciclosporin, Tacrolimus, Sirolimus, antiretrovirale Wirkstoffe, Midazolam\/andere durch CYP3A4 metabolisierte Benzodiazepine, durch CYP3A4 metabolisierte Kalziumkanalblocker, Digoxin, Sulfonylharnstoffe, Makrolidantibiotika. <strong>S\/S: <\/strong>Nicht empfohlen, Empf\u00e4ngnisverh\u00fctung.
