Kinder und Jugendliche:<strong> <\/strong>PAH der WHO Funktionsklasse II und III; <strong>D:<\/strong> Orale Einnahme: Tablette oder Granulat zur Herstellung einer Suspension. Erwachsene: Anfangsdosis: 0,5 mg oder 1,0 mg 3x t\u00e4gl., schrittweise Dosiserh\u00f6hung um 0,5 mg bis zu max. 2,5 mg 3x t\u00e4gl. Spezielle Dosierungsanweisungen: Vorsicht bei \u00e4lteren Patienten (\u226565 J), bei Patienten mit m\u00e4ssig schwerer Einschr\u00e4nkung der Leberfunktion (Child Pugh B), bei leichter, m\u00e4ssig starker und starker Einschr\u00e4nkung der Nierenfunktion (Kreatinin-Clearance 80\u201315 ml\/min), bei Patienten unter stabilen Dosen starker Multi-Pathway-Inhibitoren f\u00fcr CYP \/ P-Glykoprotein (P-gp) und Brustkrebs-Resistenz-Protein (BCRP), bei Rauchern. <strong><em>Weitere Indikationen und die dazugeh\u00f6rigen Dosierungen f\u00fcr Erwachsene und p\u00e4diatrische Patienten entnehmen Sie bitte der Fachinformation;<\/em><\/strong> <strong>KI:<\/strong> Schwangerschaft und Stillzeit; Kombination mit: Nitraten, Stickstoffmonoxid-Donatoren, spezifischen Phosphodiesterase-5-Inhibitoren und nicht-spezifischen Phosphodiesterase-Inhibitoren, Stimulatoren der l\u00f6slichen Guanylatcyclase. Pulmonale Hypertonie mit idiopathischen interstitiellen Pneumonien; <strong>WH:<\/strong> Pulmonale Venenokklusion. Erh\u00f6htes Risiko f\u00fcr Blutungen allgemein und aus dem Respirationstrakt. Gef\u00e4sserweiternde Wirkung, Blutdrucksenkung m\u00f6glich. Erh\u00f6htes Osteoporoserisiko. Nicht untersucht: Kombination mit Endothelin-Rezeptor-Antagonisten oder Prostanoiden in CTEPH, bei Erwachsenen und 12 bis &lt;18-J\u00e4hrigen mit systolischem Blutdruck &lt;95 mmHg bei Behandlungsbeginn und 6 bis &lt;12-J\u00e4hrigen mit systolischem Blutdruck &lt;90 mmHg, stark eingeschr\u00e4nkter Leberfunktion, Kreatinin-Clearance &lt;15 ml\/min und Dialyse-Patienten, bei Kindern und Jugendlichen unter 18 J. mit eingeschr\u00e4nkter Leberfunktion oder Nierenfunktion. Tabletten enthalten Laktose. <strong>DDI:<\/strong> CYP3A4-Induktoren, starke Multi-Pathway-Inhibitoren f\u00fcr CYP und P-gp\/BCRP und andere starke Inhibitoren f\u00fcr CYP und P-gp\/BCRP, Protonenpumpeninhibitoren, Antazida. Erh\u00f6htes Blutungsrisiko bei Anwendung mit Antikoagulanzien, Thrombozytenaggregationshemmern.
